Noteworthy for its publication in NEJM, this ECOG study chose survival as an endpoint in doubling the dose of daunorubicin to 90 mg/m2 x 3 with cytarabine at 100 mg/m2 x 7 for induction in AML adults under 60, using GM-CSF, and proposed transplant consolidation. Better CR rate (71% vs 57%) and overall survival (23.7 vs 15.7 months) were attained with the higher dose with comparable early cardiac toxicity. Interpretation of these data is difficult because only half of patients received planned consolidation and no data are given on relapse-free survival, the role of allogeneic or autologous transplant, or the effect of GM-CSF.
