Half of patients with advanced urothelial cancer are unfit to withstand cisplatin-based chemotherapy because of impaired renal function, poor performance status, or comorbidity. Carboplatin-based regimens are less nephrotoxic than cisplatin therapy, but they are slightly less efficacious. Because chemotherapy dosages are derived from studies of fit patients, those who are unfit and older may experience increased toxicity if treated at standard dosing.
